Posted By Chris Cooper-Abbs Dear All, Working recently with a local authority we are working towards updating RA, LOLER records etc, one thing i would like to do is question the employees (nine) on there perceptions of how H&S is handeled and what could they suggest or would like to see improved etc. Has anyone any templates on similar questionnaire or anything along these lines, comments, suggestions welcome or any links, document reference appreciated. Anything via email - great.

The repertory grid can be used for this purpose. You can learn to use it from 'An easy guide to repertory grids' by D Jancowitz, John Wiley, 2003.

Posted By Raymond Rapp Chris The HSE have a Safety Climate Tool to measure the prevailing safety culture via an in depth questionnaire. You will find it on their web site. However, for just nine people it would probably be more realistic and interesting to devise your own bespoke survey. I did one some while ago and if you wish I can email to you direct as an example.
