Hi all
We want to start doing medicals for all new starters and anyone new to FLT's (and over 40's). Does anyone know where I can get a sample medical questionaire?

I would agree with Steven. I recently set up a OH system and the OH provider ran the Medical Questionnaire system and also then took the decision for anyone to be given a full Medical based on the answers to the questionnaire and the job/task to be performed if they got the job.
It therefore take the H&S person out of what could become a very difficult situation.
I also had to set up Prescription Safety spectacle testing as part of a PPE drive and in addition anyone who required to be screened for DSE Safety Spectacles was screened and then the decision was made by the Optician as to who had to go for the full eye test. This would be predominantly people of a certain age but I deliberately didn't make a judgement call prior to this happening.

Have to agree with Steve here - I do detect a faint wiff of ageism. Why over 40s? Apparently we are meant to be able to work well into and possibly past our 60s these days (if you believe the pensions people). Of course I might just have lost the plot ( and my marbles) being all of 49 myself.

If you look in HSG(6) medicals are advised after the age of 40 to be carried out every 5 years and after age 65 every year.
At the end of the day we want to ensure that whatever job a person is given it won't make their condition worse.
I think we are being very responsible towards our people don't you??

I agree with Bob that frequency of medical examinations should be commensurate with risk, and be based on sound advice such as HSG.
The suggestion that this approach is discriminatory or ageist, is just another example of politically correct paranoia.

Like it or not as from October 2006 such ageist health questionnaires will be considered as discriminating on age grounds. See the Government website www.agepositive.gov.uk.

HSG 6 makes a valid point but it should not be taken in isolation. We have to ensure medicals are undertaken for all employees who require them as identified by a risk assessment.

Hi Bob,
We have been using pre-employment medical questionairres for many years, but only by our nurse who then states if a person is fit/unfit for their proposed role.
They are required for HGV's, recommended for FLT's and I would advocate their use on any large equipment where good hand/eye/physical co-ordination is necessary for safe working.
Have you considered what to do with anyone who fails to meet the grade. Suggest you agree your policy on this before proceeding
