Hi All,

at present the company i work for have no accident report form. I have been assigned the task of creating this and have been looking for a template to work off to ensure that i capture everthing needed.
If anybody has one could you let me know so that i can get you to send it onto me.

I have one that I ran passed the DWP in light of the Data Protection Act and they have given it the okay! If you like you can have a copy of this form?
