I have qualified to audit to the above. As a starting point I intend to conduct a "gap analysis" type audit on our organisation. Does anyone have such a document I could make use of? Many thanks in anticipation.

Surely if you are qualified to carry out this audit you will automatically identify the areas that require attention?
This information will form your gap analysis report.
Why do you need a form for this?
