HSE guidance on HAV'S suggest a simple questionnaire would be adequate for Health Suurveillance.
I assume asking, do you suffer from any of the symptoms. If the candidate answers yes, what next!!
Do we not employ or lay off this person?
When replying please be mindfull this is in the Construction industry. We have very few PAYE.

I would suggest establishing if you have a problem first ie look at vibration levels for the equipment you use (available from the supplier but double this as it's lab based!). Get employees/people working under your control to keep a record of what they use and for how long over a period of time. Review your maintenance of the equipment to keep vibration levels as low as possible and replace with low vibration equipment where practicable......
THEN, for the people who are likely to be at risk from HAVS and yes this might be everyone but it's unlikely to be, carry out health surveillance. It can start as a questionnaire but you should keep your procedures afterwards transparent to try and get honest answers. This questionnaire should be overseen by someone who knows what answers to look out for. People that flag up as having a potential problem should be referred to a qualified Occ Health professional to determine the extent of their symptoms. They should be able to provide advice on how this individual should be treated. For some this may mean no more work with vibrating equipment and alternative work found where possible. For others it can be managed effectively by controlling exposure.

Good response from hjy. However, as Christopher has identified, there are few PAYE operatives exposed! This means that there is a considerable incentive for non-PAYE operatives to avoid telling the truth for as long as possible as the consequences will be an enforced lack of earning potential!!
Nothing new here; just a reality of many different work areas where those at greatest risk are not direct employees.
The real issue is how to get the non-PAYE operatives to give honest answers before they are visibly affected. Sorry, I simply can't provide a simple answer to that; and if anyone out there can - let us all in on your secret please.
