Hello all

Does anyone know where I could pick up a dvd's on near miss reporting, risk assessment and construction site safety. They are for taking to sites where our company work with either a laptop or portable tv (pat tested with manual handling risk assessment in place!). A near miss reporting dvd would be of particular interest as we are currently implementing a near miss initiative.

Also to add to the above I am putting together a near miss reporting form for our workforce to hand in (M&E installation operatives on new build construction sites) does anyone have any existing forms I could look at?
