Risk Assessment worksheet for engineers to complete

I am hoping someone can help me. I am looking for a one page risk assessment work sheet for engineers to complete prior to carry out work on site. (M & E industry) Anything to give an idea of layout and format would be very much appreciated.

This is an area that is very familiar to me and firstly I would comment that the worksheet is more a system of controls and checks that spring from your top level risk assessment for the activity. Personally I would suggest that if possible you avoid the words risk assessment - because it is not within the title as it causes confusion. I would suggest as an alternative to worksheet you call it a risk assessment check list.
This more like significant findings that spring from your assessment and a method of recording that the engineer did in fact do this BEFORE starting their work and carry out the control measures. In fact it could be that your engineer would in fact not start a job if some of the requirements cannot be met. This means that the engineer needs to be fully aware of the content of the original linked risk assessment in order to understand the importance of the check sheet.
There is room for dynamic risk assessment for your engineers who should be competent enough to recognise the hazards before you send them out in the field.
Any way in the spirit of this forum I will send you some examples to start you in the correct direction provided you make them yours. They represent a considerable amount of development and are not for general distribution for any course that you may run.

This one would interest me too. I'm currently on a project to teach dynamic risk assessement to maintenance people. Have lots of info for police and fire brigades but need to tone it down a bit. Any help/advice would be welcome.
I rarely ask for advice here. Prefer to offer it. But DRA is a fascinating subject with lots of applications.

We are currently discussing the implementation of Dynamic Risk Assessments for Engineers out in the field or completing tasks that are not text book. Can you give me any advice and guidance on training, format, checklist etc. and how you have applied them to your application.

I have e-mailed you a copy of the check sheet we use for our engineers.
This is based on working outside only, so may not be much use to you.
The check sheet is linked to the main risk assessment - engineers know what to do when they come across various hazards.
