Do not re-invent the wheel. Get hold of a company near you or like you and ask them what they have and see if you can crib/copy/learn from that.

Get a copy of HS[G] 65 - Successful Management of Health & Safety and read it thoroughly. It's cheap & readily accessible for peanuts from HSE Books.
After that, get hold of a competent H&S professional to assist you to turn it into reality that is relevant to what you do.
