Posted By Frank Hallett Hi Beverley I'm sure that I've seen something like this on this Forum a few months ago. Give us a clue about the context, relationships and required outcomes please 'cos your form will have to be client relationship specific to be even marginally meaningful. For instance, a client satisfaction form for Home Helps will require two very different strategies - one for the Home Help and one for the client. It is entirely possible that this should be contracted out to an independant [like me!]. Frank Hallett

Posted By Charley Farley-Trelawney I may be able to assist; as Frank rightly states, it would be prudent to provide a little more information to answer your question efficiently. I have survey forms that are applicable to organisations enjoying our facilities, contractor surveys etc etc, it maybe that one may be suitable with some mild re-arranging. All the best C
