Fraudaulent employee claims - Disciplinary action

The case of Hughes v Caerphilly County Borough Council has recently been brought to my attention. Although neither Hughes nor his witnesses were employees of the Council, the fact that they were found to be in contempt of court for making false statements leads me to ask the following questions:
Are fellow professionals aware of organisations that have taken disciplinary action (or have a policy) against employees making (or acting as 'witness' supporting) fraudaulent personal injury claims?
Are fellow professionals aware of organisations that have taken disciplinary action against employees for not complying with their S7 responsibilities where their acts or omissions have had material consequence in a personal injury claim pursued by another employee or member of the public? (Or have a specific policy that states this)

Hi,
No, not directly. But we would, I am sure, in the circumstances you describe. I have worked for an employer that hs disciplined somebody for S7 reasons, but it wasn't connected with insurance fraud,
