Posted By Saracen11 Hi JAI, I always base my investigations around the ABC principal:
Antecedent – what happened before the incident and what was involved – if equipment, ask the usual questions about servicing and maintenance, correct use of, training, compliance with legislation, and reference to the manufacturers instructions for use. Include looking at any other similar incidents.
Behaviour – what was the IP doing immediately before the incident, at the time of the incident and immediately afterwards. You’ll also need to know what everyone else in the area was doing at these times – don’t forget to cross-reference information with accounts of the environmental factors i.e. noises, lighting, distractions…
Consequences – immediately identify what you need to do to prevent the situation from worsening. Identify the impact of the incident in terms of the effect on the IP, the witnesses and the culture within the workplace. Include damage to equipment, effect on production/lost time etc… and possible breaches of legislation - prepare your defence ready for the visit from the enforcing authorities/civil action.
All of the above will enable you to determine the cause, what actions you should take to manage the outcome and how you should prevent a reoccurrence.
