I would appreciate advice on work permit for diving operations and precautions that should be included.

It depends on where the dive is taking place. Is it on a vessel, installation or a civils site?
If a vessel its basically a lock out tag out confirmation, including:
Intakes
propulsion systems
outlets
cathodic protection systems using impressed current
etc, etc, etc.
Permit should also include confirmation of Alpha flag flying at work site, methods of communication (VHF channel etc) start and finish time of permit.
