The domino theories I know tend to assume that, as you can't change anything at the strategic, tactical or operational levels, the most effective "domino" to aim at is employee behaviour. The following and last domino is usually "defence mechanisms" (how do we prevent injury despite our stupid employees) The latest one I have on the bookshelf is Cooper, 1997.
This is the basis of current Behavioural Based Safety (BBS)
Not being a practising, chartered behavioural psychologist I am allowed to disagree without producing either evidence or statistics.
Therefore I can safely say that action and change at the strategic (Board) level (the first domino) will be the most efficient, economic and lastest longing of any other choice.
But you try to get the (deleted)s to listen to you.
