well "worker" is right but...
of course Management are
responsible for accidents.

They are in control.....or lacking it?

I am sure there are others with alternative
views on this, please let's discuss?

Whether management are in control or put another way, accidents are the result of management failure is largely a matter of who you prefer to believe. Some people advocate that up to 80 percent of accidents are caused by human error, whereas others state most accidents are caused by latent failures. I suspect the truth lies somewhere between the two doctrines. I prefer to adopt the view that the 'immediate' cause of most accidents is human error and the 'underlying' causal factors are often management failures.

Who is responsible will often be a matter of conjecture. After all, reality is socially constructed.
