I'm trying to find further details or links to an incident in the NHS that hit the media either early this year or last year.
A patient died as a result of a radiologist not spotting cancer on an xray or CT scan and consequently the patient did not receive treatment.
I think the areas of concern was related to not having a double checking system. Rochdale Hospital rings a bell but I could be wrong.

I've googled it with no results. If any help would be gratefully appreciated.
