Posted By Martin Taylor Is there a requirement for the first aider or appointed person dealinjg with an accident to complete the Accident Record Form (the official 'accident book' sheet) I am used to this being the process but have joined a new company where the responsibility is put mainly on the IP. What is the norm? Martin

Posted By Vicky Godberford-Orton Martin, The IP normally fills out the form and it is then signed by the Manager/Supervisor. If IP is unable to do so, the Manager/Supervisor does so on IP's behalf. The section whereby a first aider is mentioned: 'Was any first aid treatment given?' 'If yes, state what' The first aider may be required to fill out this section if necessary. I assume this is the norm, however other Companies may have other set procedures. Hope this is of use. Vicky

Posted By Ken Taylor If we are talking about the book that was once the BI510, the IP is entitled to fill in the form to record their accident - although another can do this on their behalf if they are unable to do so themselves. The F2508 form is for the employer to complete and as to any other local company accident recording procedures it's a matter of local policy.

Posted By darren booth i am a first aider,as well as a safety rep. i allways fill in the accident book as a matter of course, with the injured party present. i work in engineering ,and the majority of injuries are cuts to hands- we dont want blood all over the book! on a more serious note, not all employees have the same standards of literacy,and the entries need to be legible. we try and look at the injury records annually, as well as monthly, to try and identify problem areas and it is difficult if you cannot read the entry!

Or maybe you will not enter details the injured party wants entered ? Lets reiterate this: The information entered is not what managers or safety reps want entered, it is what the person injured wants entered. Sounds a lot like a company I know of, the production manager always entered the details himself, and the details entered were what he wanted entered. It took an anonymous complaint to hse and an inspectors visit to get things sorted (and to get the guards re-installed on the machines....Oh, and he isn't any sort of manager anymore)
