There are a number of specialist OH providers most of which are are on the net and which one you choose would depend on your location.
I would be interested to learn what 'guidance' you are quoting. For example, I recently applied a health questionnaire etc to staff who work nights and did not consult a OH provider. My company does employ one for various circumstances.

The 'guidance' i am referring to is for the working time regulations. A copy can be sourced from the DTI website, and if you go to section 9 it is emboldened "This health questionnaire is provided for sample purposes only. Make sure you ask a qualified health professional to help you devise this form"

Posted By Raymond Rapp Aah, I see now. Personally, I would not give much credence to the DTI caveat, it sounds like a bum covering exercise to me. What do the DTI know about health and safety anyway?
