Posted By David Bannister Forgot to add that I particularly liked the one from New Zealand about the rear seat passengers in the car sniffing from an LPG cylinder whilst the driver lit a cigarette - this is an assumption from the bits that were found!

Posted By Mike I don’t have any personal experience of the notorious torpedo effect where the neck of a high pressure cylinder breaks off in a fall. This may or may not have happened in modern times. I have experience of a potentially fatal incident in a lab where the wrong regulator was screwed onto a helium cylinder by an person with well below normal finger strength. The thread match was close but not close enough. With their weak fingers they could not sense the cross-threading and forced it with a spanner. When the main valve was opened by a second person the regulator flew off at very high speed. Only by the chance orientation of the regulator was a fatality avoided. In the event the second person suffered temporary deafness and psychological trauma. There was a chlorine incident I witnessed in the early 1970s at Salford University. A chlorine cylinder regulator valve froze solid in the open position and operators broke the valve trying to force it close. Fortunately it was on or near the ground floor and firemen with BA eventually heaved the leaking cylinder through a window onto a lawned area outside the lab where a white circle of bleached grass appeared.

My story is about the gas pipes connected to the gas cylinders.Which i was lying on un- conscions for 3/4hr,which had reignited in the area i was working in.It was the smell of cooking meat that brought the attention of my saviour (poor Billy)and raise the alarm. The rest is history.I now give H&S seminars on what happens when it goes wrong and how this accident could have easily been prevented. Posted By lewes I have a couple of pics with a gas cylinder in a car which were not secure. Both ended up through the front wind screen. Also got a newspaper clip of a fire and the aftermarth of a gas cylinder now looking like a banana skin !!!
