I'm looking at getting some health and safety related questions into our employee opinion survey.
I've got some ideas for questions that will ask about employees perception of how seriously we take H&s and whether or not they see it as a barrier or help in what they do.
BUT .... I would like some other ideas if possible, because I'm sure I won't identify all the good questions to get an idea of their perception of H&S

I personally only ever ask one question:
What is your opinion of the H&S culture within the company ?
I do not give the option of good, bad, indifferent etc, I wait for the full length answers which gives a far greater insight IMHO.
