I want to start recording my Near Miss reports in some for of spread sheet which allows me to see where we are on our trends (pie charts and such likes) I'm getting on a bit and not into setting all this type of thing up

can anyone by per chance help me out on a format to colate this info

im in the construction game if that helps

You could make use of MS Excel or Access. Access would be the easier option.

within this software you can make reports based upon inputting a query against the data collected.

To group near misses by type a drop down list can be allocated to a cell. Here you could select the type of near miss, such as vehicle, fall, slip, operator error, etc...
