Posted By David Bannister May, in my opinion you need to be absolutely certain that the supplier has made no other changes to the product (formulation, dilution, strength, potency, physical form etc) that will alter the risk to anyone likely to be exposed. If this is the case then simply note the name change. Any change that affects how the product is used or likelihood of harmful contact must be dealt with by a review and change in controls if needed. In any event, reviews are needed from time to time and especially when there is a significant change in risk. If the assessments are coming up to review time why not bring it forward?
