Need more clarity on what file you are talking about.

Is it the health and safety file issued at completion you refer to?

If so just go to page 62 of the new ACOP, why reinvent the wheel, just use this list as a guide and adapt to suit the nature and complexity of the project / structure.

There is further industry guidance on the new CITB website link on CDM industry produced guidance that you may find useful.
