Can anyone advise me as to the criteria i should concern myself when making a hot work permit?

Any help would be greatly appreciated.

Important point : Permit should only be authorised after verification that the work area has been corectly prepared.

Permits should only be completed and signed at the workplace. never in an office or control room. Sacking offence for the H&S person according to my old boss.
