While writing a report in support of adequate health promotion and sickness absence management programmes, I would like to quote examples of accidents caused by persons working while unfit for work. Can anyone provide me with examples (preferably with published references) of such instances?

Helpful certainly, but I would really like a big meaty catastrophe with lots of damage and employer liability to drive the message home.

I have found one of my own - train driver heart attack causing crash in Australia.

However there are already medical fitness standards for drivers.

Perhaps I am not going to find out that the control room operator at Chernobyl had the flu but soldiered on due to poor company health care provision.

What I would like is other evidence of poor medical care / provison for staff leading to incidents, accidents and other negative consequences.
