While writing a report in support of adequate health promotion and sickness absence management programmes, I would like to quote examples of accidents caused by persons working while unfit for work. Can anyone provide me with examples (preferably with published references) of such instances?

Perhaps I am not going to find out that the control room operator at Chernobyl had the flu but soldiered on due to poor company health care provision. What I would like is other evidence of poor medical care / provison for staff leading to incidents, accidents and other negative consequences.
