Examples of permits are usually at the back of this document, well they were 10 years ago!

I would recommend you get both HSG85 "Electricity at work safe working practises" and HSG47 "Avoiding danger from underground services" both available from HSE Books and easy to read!!

Before issuing any permits to work or permits to excavate.

The first publication contains a sample permit to work and i can email a permit to dig.

The permit issuer must be competent (i.e know about electricity / excavations)and they should follow readily available guidance.
