Homer, not a one-pager as such but the following may help:

Minor incidents are often a precursor to a more serious incident e.g. domino effect.

A proactive safety culture is often epitomised by the quality of incident reporting and investigation e.g. learning from your mistakes.

Reported unsafe acts and conditions that do not result in an incident (near miss) are a free lesson in which to learn the underlying and immediate causes.

But don't believe slavishly all you read.
