Hello, I'm after a bit of advice...

The HSE have asked us to conduct a human factors assessment of safety critical tasks at one of our top tier COMAH sites.

Has anybody else conducted such an assessment, and if so, would you be willing to share the results (or at least a blank template of how you went about it)?

I've found plenty of information on the HSE website, but I'm struggling to capture and present the relevant information in a succinct way.

Any useful pointers would be gratefully received!

Since you're talking about a high-hazard situation, the HSE will reasonably expect you to understand the essentials of ergonomics. This is best done through well-chosen reading if you haven't worked closely with a registered ergonomist.

The method you need to follow is shown in the HSE document at http://www.hse.gov.uk/humanfactors/comah/core3.pdf

I'd suggest you get some help to get started as it can take a while to work out where the focus of effort should be. You have my contact details.
