around 5 years ago i was in an environment where upon the work force came across some asbestos we were all then given a form that said we may have been subject to asbestos contamination so that in the future we were able to say this is were it was from
so to cut a long story short my wifes mates husband has just reported a firm that he works for to the h and s as the firm new they were sending them into a contaminated field to clear when he sent his overalls away for testing it was covered in the stuff the firm had to then stop production at there premises until the site awas clear and the firm are now trying to blacklist him as a contractor with other firms as the closure cost them big time but my question is do you still get a form to say you may have been subject to asbestos from the firm for future use ????? your answers would be most appreciated

Not as far as I know - I've dealt with a few mass exposures and never heard of it - It may have been something direct from your old employers insurers to say they were liable... Sorry can't help you any more than that

I would think it most unusual for a company that was (by the sound of it) deliberately flouting the rules, to give its employees a piece of paper saying that they had exposed them to asbestos!
Not that it matters. If the point of the request is for your wife's mate's husband to retain proof in the future that he has been exposed to asbestos then surely all he has to do is keep a copy of the results of the tests carried out on his overalls. Additionally you imply that someone official has now been involved (it's not clear exactly who you are saying has been involved - HSE?) so they will also have a record.

In accordance with the MoD Health & Safety Policy (JSP 375, vol 2, leaflet 5), staff who have worked in environments where asbestos may have been present and are concerned that they may have been exposed to asbestos have the right to have that fact noted on their personal records.
Any concerned staff who should complete MoD Form 960 (Asbestos Personal Record Annotation) and forward it through their line manager to their Personnel Section. In the case of retrospective recording for former staff, the MoD Form 960 should be forwarded directly to the appropriate personnel authority.
i thought it was policy throughout and that is why i asked as asbestos is not my thing but exposure to it makes one concerned

You should refer to para 251 of L143 - the ACOP on the Control of Asbestos Regs 2006 which says..."For any employees who were not wearing adequate RPE or have been potentially exposed to asbestos fibres in an incident, a
note that the exposure has occurred should be added to the employee's health record or to the employee's personal record if they do not have a health record. A copy of the note must be given to the employee with instructions that it should be kept indefinitely".
The spirit of this is for accidental exposures but nonetheless I would suggest that your friend was "exposed to asbestos fibres" and should therefore have the right to a note. You could ask your local HSE office for further advice.
