Posted By AlisonSM I'm looking for a DVD that I can present to staff on how to conduct an accident investigation. I'd really like it to have an office scenario rather than an industrial one i.e. person trips over a cable in the office and breaks an arm..looking at the events leading up to the incident, how to complete an accident form and what control measures could be implemented etc...
Does anyone know of such a DVD and where I could locate a copy (or be willing to provide me with a copy).

Posted By AlisonSM I've completed the Rospa Accident & Investigation course (which has DVD presentation with an incident to be investigated I might add).
I would not be "just" showing the DVD but going through a full risk assessment (who, what, where, when etc) plus control measures that could be implemented. The DVD was purely to show what could possibly go wrong in an office and make the attendees think about where they work and the types of risks present.
