Does anyone have any learning from isolator failures, where the casing has exploded?

Yes.
Only when the isolator has been massively underated, protective discrimination has been totally ignored---------ie, rust nail scenario, or as a result of a direct short circuit on the phases.

For a 400V isolator to explode, there would have to be a very large fault current. The short circuit protection would also have to be unsuitable or faulty. Occasionally, they might fall apart when operated and short out. There is a risk of explosion should the enclosure be full of dust, from grinding for example.

Is this something which has happened?
