You may or may not be able to help on this. Having reviewed accident data/stats for the last 4 years Finger/hand injuries were by far the biggest contributor at our site.
Over the 6 month period there are 14 (lost time >1 day of those 3x serious) finger/hand injuries.
Accident investigations have pointed out root causes such as:
Failure to use gloves or use improper gloves
Use hands instead of getting a proper tool – i.e. hitting with the palm of the hand, probing with fingers when fault finding, etc.
Misjudge how hot an object is.
Don’t think about the “line of fire” in the event of an unplanned incident.
Despite safety awareness TBTs and various activities we are still having these type of injuries.
With out getting into too much detail a new look/perspective is needed we are now trying to develop a Safety Campaign to raise employees awareness i.e. to think about the protection of hands and fingers whilst undertaking work activities - every aspect of safety management needs to be looked into not just task, equipment and design but human behavior (factors), we have some very good ideas for the campaign from our employees but I am also calling for some ideas from IOSH members.

I was one of the presenters on hand protection for the British Occupational Hygiene Society's Personal Protective Equipment Special Interest Group's seminar 18 months ago on gloves and hand protection. Whilst my main concern is chemicals and the skin, my presentation also covered physical hazards.

Great to see evidence of such a thoughtful approach to influencing awareness of high-frequency risks on the part of employees and line managers.
Since you ask for additional perspectives, may I suggest that you give equal attention to positive consequences of reducing and then eliminating accidents. Simple stuff about positive motivation but it influences lots of kinds of behaviour including care about using hands in ways that result in lost-time accidents.
