Posted By lewes Morning All Not being a first aider myself I didnt know we needed to have them, but has anyone got a template First Aid report they use when treating an injured person in the work place ?? Thanks for all your help lewes

Posted By Coshh Assessor You don't need to have one. The first aider can simply record the treatment / advice on the accident form.

Posted By Bob Shillabeer The only information you need to record on the accident report is the fact that first aid was given and the casualty either resumes work, is sent to hospital or goes home. You need not record anything else or you may compromise the first aider if any other sort of injury is subsequently found. First aiders are generally volunteers so it would be very unfair to put these people in this situation. The company accepts any responsibility for a first aider who has been propoerly trained and acts on behalf of the com[pany when acting in such a role whilst at work. All you need to record about firat aid is that it was delivered by a trained company first aider.

Posted By Coshh Assessor As a first aider I have certainly been advised by first aid trainers to record any treatment / advice I give - for my own protection in case it is questioned later. I don't see any advantage in not recording this information.

Posted By RoseR There's no reason why you can't have a book in the First Aid room that asks for Name, date, time, why the person came to see you, treatment given, was it the result of works accident (y/n)column. It's good practice as a first aider to record any treatment you give to someone. I've been asked to complete reports on accident forms before and a simple paragraph such as 'person had cut on index finger, right hand approx 2 cms long. Cleaned cut, covered with plaster. No swelling or brusing noted at the time. Person returned to work.' Ask the First Aider to record what they saw, what they did and stick to facts.
