On section 15, the declaration, there is a box marked "role".
What role would the person signing on behalf of the client be expected to hold.
Also
In section 1 they give the address for the F10 scanning centre, but not for operational railway work. Why is that?

Use the old form it is still legal tender whatever the HSE would like you to think. The question of role is then irrelevant - The signature is by or on behalf as required by regulation 21 and the appendix.

Railway F10s go to the relevant enforcers ie the Railway Inspectorate. The HSE does not enforce railways.

But do not get me going on this incredibly stupid form intraduced without proper consultation with the industry.
