If you seen the accident yourself write down exactly what you seen do a sketch of the area take pictures of the whole environment it took place. Who was involved in the accident where proper procedures in place who's fault was it then you start into the Health and Safety regs part of it.

State the facts and not opinions and don't use emotive language. Be balanced in your approach as a report that just states what breaches occurred could lead to enforcement action and almost certainly a large pay out when a civil claim comes in.
