Hi All,

May I know where to get any template for hazard report form? Something where employees can report a hazard so that they can fold like a small leaflet? Maybe 50% of an A4 paper where fold into half.

The form seem too big where in A4. I'm trying to find some that is 50% of A4 size. It then can be fold up & deposit a letter box.

Something like "Safety Observation Scheme", "Hazard Report Form" similar kind of name.

Hope you can share more such kind of link website.

most of the sites i work at have these.

You do not need a template as such. Just create a simple form on word or excel to the size you require.

It only needs to be very very simple.

Set the print setting to A5. Then print the form on A5...!!

Copy and paste in landscape, you then have 2 copies on one sheet. Cut in half, 2 for the price of 1. I do.
