When report writing for a client do you include a brief résumé ?
The reason for my question is:
Recent report sent to client, reviewed by HSE inspector, then phone call from said inspector asking about my qualifications/experience etc. Not unhappy with that, fair comment I think, until I was aksed if I was trained at "Woolworths"
I would appreciate any comments (not those that would like to question my credentials) !

You could include in the report the principal qualification of the author or alternatively include your post-nominals after your name. These would then give the reader a clue as to the 'competency' of the author.

I don't include as resume but my signature includes my post-nominals.

I'm not a consultant but think the said inspector should have asked your client and not you.
If I was a consultant I wouldn't put this in every report (you've been selected to do this work on your competency and therefore don't need to keep repeating this).
Inspector was rude to Woolworths safety people (or all now ex).

To a forty or fifty page report I always include a 1-page "executive summary"
Starting with the high positives then the low negatives.
And I've got this darling rubber stamp which gives my IOSH qualification and number.
But still says MIOSH, Not CMIOSH

Only include CMIOSH MIIRSM. Clients ask for my CV in advance of my work sometimes.
I'd take it in your stride. Many HSE inspectors just don't like consultants. Take heart in the fact that you may be more qualified and more experienced than them!

It is unprofessional and discourteous. I would make a complaint to the Principal Inspector.
He may have an issue with something you have written but there is no need for such childish behaviour and one-upmanship.

Not wishing to hijack this thread but, someone from head office completed a form on my behalf as the 'employer' and included in his post-nominal IOSH and OLSH, never heard of these, nor did he include his membership number. Hence the form bounced back. Strange...
