That will depend on what sort of health and safety questionnaire it is - i.e. who is being asked to answer it and what you will use the answers for.

The whole work force will be asked including the employees of the sharing company. The answers will be sent to head office as they carry out questionnaires from time to time and aways give a good break down of the information.

Is it mandatory? If not, and you get a low response rate, you might especially need to 'validate' the data you do get, e.g. through focus groups.

I have started this action with the mind set that the first one will give us a level to start with to build on. The safety culture should be highlighted with this so if it gives us a dark picture then we can create an action plan to fix any issues or reduce there impact.
The H&S committee will be involved as will other groups.

Have to say that, in my experience, they're one of the most unreliable ways of gauging culture, understanding, whatever.
And whoever wants to can emphasise what suits them from the results.
They infuriate me.

To be hinest I don't like questionnaires, they are so subjective.
They depend on subjects motivation, honesty, memory, and ability to respond.
Subjects may not be aware of their reasons for any given action.
They may have forgotten their reasons.
They may not be motivated to give accurate answers, in fact, they may be motivated to give answers that present themselves in a favorable light.
Structured surveys are even worse particularly those with closed ended questions, which have low validity when researching affective variables.
