I have tried looking for a standard questionnaire but I can't find one. Posted By Chris Packham Angela Have been away for two weeks, so unable to respond until now. There is more to skin checks than meets the eye. A questionnaire or check list is only an aid. As I don't know how much you already know about the skin and whqt I assume will be a visual check, you should be aware that it is easy to look at the skin and reach a false conclusion about what you are seeing.
