I need to carry out a task analysison an individual at work who, after having an accident at work, can no longer carry out his normal duties.
Are there any templates i can use to put this info on or have any of you had to conduct one of these assessments??.
I am looking at putting step by step instructions.
weight of equipment
frequency of activity
parts of the body used.
any help would be appreciated

Now i have a little more time....
I need to compile these assessment so that they can be sent off to an occ. health doc.
He can then make a decision on which duties the person should be able to cary out without increasing the risk of worsening his existing disabilities. Also so that it doesn't cause him any discomfort whilst carrying out his duties.
I have also included on the assessment sheet:
PPE/special precautions
Haz. Prob. Freq. Risk score.
A comments box
In addition to what i mentioned above.
The company as a whole hasn't really got a standard template to work from, so they will be implementing mine as the template to start with. I could do with making a good impression.
I was considering entering torque forces used and obtaining these measurements, where possible, by using either manufacturers recommendation or by using a torque wrench on the existing fittings to see what they are torqued up to.
Any other help, advice, ideas would be most welcome.
I shall be using the template in an excel format

I wouldn't get too complicated with the template. Firstly as a template you need other people within your organisation to be able to easily understand what they are supposed to be completing, secondly the occupational health guys are unlikely to understand the complexities of torque in relation to disability.
The other stuff you've mentioned sounds reasonable. As an idea why don't you give your occ health guys a bell & ask them what sort of information would be most useful to them in order to provide you with a sound assessment of what this guy can reasonably achieve.

I have put together a very similar thing for commercial vehicle drivers returning to work and the same principles could be applied.
Take note from the previous mention of occupation health - they will need to be brought with you in the 'journey' (excuse the pun)...
If you would like expand on what areas you are looking at or are you just looking for a template to return your assessment in?

Apologies if I'm stating the obvious and/or you've already thought of it, but is there any reason why the occupational health professional can't come to the workplace, meet the individual, see the tasks that he'd normally perform and then make recommendations?

We have occ. health nurse that comes in once a week, it is her who is requesting this.
She isn't qualified enough to make decisions on what long term tasks he can do. That is why i need to carry out this assessment which will be sent off to a occ. health doctor. He will them make a decision on which activities will be suitable for this person to do.

Sounds like an expensive process with you Occ Health... The nurse cannot help in completing an assessment with you that is passed onto the medical specialist?
I take it you are paying per visit etc?

I have looked at a few and used the REBA (Rapid Entire Body Assessment) and RULA (Rapid Upper Limb Assessment)tools (depending on what they are doing) as you analysis the way they do things and the postural aspects which may give rise to risks.

You need to provide to the Occ.Health Doctor brief details of the tasks the individual is likely to carry out, along with all and any appropriate Risk Assessments. Where physical capacity is affected, the checklist from the old style Manual Handling Assessments might be a useful tool.
The employee needs to give consent for the Occ. Health Doctor to be able to contact the GP.
The real key to the process will be the dialogue between the Occ.Health Doctor and the individual employee.
What is "reasonable" must thereafter be determined by the employer.
There is some risk involved in attempting to systematically address all of this this in anticipation of medical advice. Be wary of conducting any sort of "assessment", but do provide as much meaningful information (in your domain) as possible. At the end of the day, a 'phone conversation with the Occ.Health Doctor (after he has had the opportunity to assess the employee) will usually resolve any issues arising.
