RIDDOR - Failure of Lifting Equipment - Fork Trucks

Under RIDDOR its necessary to report a failure of lifting equipment including that of a fork truck. My question is what constitutes a failure under these circumstances?
I have a forklift were a mast bolt sheared off but the second one held. Would this be sufficient. My thinking is not but would rather be 100%

Schedule 2 - "failure of any load bearing part" is reportable.
You could perhaps argue the bolt is not load bearing (sorry, not familiar with FLT construction) but assuming the FLT has been used and examined properly etc so there is no obvious reason for the bolt to snap, perhaps there is an underlying problem which may affect others, so I would report it even if RIDDOR does not technically apply.
