Does anyone have, a Customer Satisfaction survey template I could possibly re model.
I am a construction manager and am looking at sending out a survey to customers ( which are all in house) on performace, quality, cost and safety.
Rather than re invent the wheel it would save me time if there is one out there I could adapt to suit my requirements

If you go to www.fpal.com and go to the downloadable forms section, there are some feedback forms that can be used. Although these forms are mainly geared towards the oil & gas industry, you could ammend them to suit your own needs.
