The following are two sites I used when doing this for my diploma. Not how up to date they are as not required it since:

Also if you go to the NEBOSH website and look at the January 2006 examiner's report there is an example of completing a FTA. The subject you get in an exam might be different but the process will be exactly the same.

You can also do a search on the study forum under 'FTA' or 'Fault Tree Analysis' and you will find plenty of others who have requested similar help.
