Could anyone enlighten me on how guidance notes should be structured? Is/Are there any format/skeleton you can use to generate one?

I presume you mean safe working practices or standard operating procedures?
If so, you need to cover what you are trying to achieve, how to acheive it, what outcomes/etc.
Try searching for safe working practice on google - it throws up gazillions of results.
You will get more answers if you post this on the OSH forum rather than on this careers one.
