Does anyone have a protocol for formal investigation of accidents/incidents in their workplace that they are willing to share with me?

Thanks for this it will certainly help! I am also looking for someone to share or lead me to a procedural document on how the Corporate Safety teams determine what accidents they will investigate and the process they follow.

I suspect that the subjective nature of accidents and incidents does not lend itself to any formal process per se. Ideally there should be a two or three tier stage of reporting and investigation, dependant on the severity or potential severity of the incident.

you should be able to find all that you need in the HSE book
HSG 245 Investigating Accidents and Incidents.
It may now be available free from the HSE website, otherwise it costs £9.50

The IOSH Guide 'Learning the Lessons' should also help you - it's particularly targeted at more serious incidents but much of the content is applicable to all types. Downloadable from 'information centre' to the left.
