Posted By Raymond Rapp Sharon I suspect that the subjective nature of accidents and incidents does not lend itself to any formal process per se. Ideally there should be a two or three tier stage of reporting and investigation, dependant on the severity or potential severity of the incident. Happy to discuss off-line if you wish. Ray

Posted By PH2 Hi Sharon, you should be able to find all that you need in the HSE book HSG 245 Investigating Accidents and Incidents. It may now be available free from the HSE website, otherwise it costs £9.50
