Posted By Adam Worth If the malfunction was when the set was in use (this includes pre user checks) then it's RIDDOR reportable (in the UK of course) http://www.hse.gov.uk/riddor/guidance.htm If found as part of a routine test - we used to isolate the set and get it fixed by our service contact operator. not sure what else you are looking for but I am sure people who a lot more experience than me will help and clarify.

Posted By John J Based on your brief description it is likely to be a RIDDOR reportable dangerous occurrence so needs reporting (if in UK) This will add some weight to any argument with supplier or manufacturer regarding equipment suitable for purpose. They should be able to provide you test and inspection data and also a report on why the equipment failed and actions taken to avoid a re-occurrence. You do have some questions you need to ask regarding use at your end. Its not clear whether the fault was discovered in use or as part of the pre-check. If its in use you may have questions to ask regarding competency. You also need to consider the substance the user was exposed to and whether health checks/monitoring are required. Any good supplier or manufacturer will be falling over themselves to help as this could have significant consequences to their business.
