I have numerous types, but I have to ask the question - why are you changing what you have in place already?

If you need further info or examples PM me directly (although I may not be able to get back to you till tomorrow).

If your in the construction industry ours is a derivation of one used by a member of the main contractors group, fairly long but it never gets knocked back if its filled in properly.
Mail me direct if you want a copy.

While on this subject who produces a MS that is as thick as a telephone directory containing lots and lots of useless information, that nobody wants to even look at never mind read?

Lots of them about.

My MS is a few pages and contains only info relevant to the actual task.
