Ocassionally see cases on the Safetynews website but there's no search function on it so it might take some trawling.

Call the nice folks at RIDDOR.

For UK incidents they will have data for you. Ask for the statistics team. Talk though what you require, the date range and any regional or employment critieria for selection. You can even do free text and keyword searches but expect a lot of hits many of which may be wide of the mark.

The statistics people will very quickly be able to give you printouts of anonymised RIDDOR reports that may be just what you need.

There may be a charge for the service, that varies tremendously depending on the number of matching incidents in their databases and the work involved. I must say that they have been incredibly helpful to me in my reserach work.

You have not defined whether you are assessing gaseous Ammonia, which is seriously dangerous, or ammonia solution (dissolved in water, but often called Ammonia) which are industrial / domestic sterilizing or cleaning type agents - very much less so. Suggest you define your search.
