Hi All Havent posted for a long time, im on the hunt for some examples of employee health and safety questionnaires. I have not had any experience in developing/using one so am looking for some good examples used to guage the culture/climate of a company currently operating or looking to implement a HSG65 based management system. Any examples or direction on where to find some would be great. Thanks

Since you say you want to 'guage (gauge?) the culture/climate of a company currently operating or looking to implement a HSG65 based management system', if you employ more than 50 people it's worth designing a questionnaire that suits your own company. An alternative, if you are committed to a behavioural safety management system, is to use the one made available by the companies of charterd psychologists who are also chartered safety & health practitioners, like Dominic Cooper. If you want to have a chat about doing your own survey, and would like some guidance on what books to consult on survey design and how to use Excel or similar program to process your results, you're welcome to call me on 020 8654 0808 during standard office hours

Hey The Health & Safety Laboratory have just revised and relaunched their / HSE's Safety Climate Survey Tool. All the hard work is done for you, questions written, reliabillity and validity tested (hopefully), database provided for data recording, reporting options available at the touch of a button - least that's what the old tool did so hopefully the new will do all this and more! Not sure how much it costs, but the old one was quite cheap about £100 per copy I think. Have at look at HSL's website.

TCG Hi, Loughborough University have developed a good survey tool (originally for the off shore industry, but equally usefull to many heavy industries), Its free to use, and I believe all they ask is that a copy of your findings are shared with them. for statistical analysis. http://www.lboro.ac.uk/d...s/bs/safety/document.pdf Regards Simon
