I am looking to further develop our strength in incident investigations and want to organise root cause analysis training within the company. Has anybody any relevant experience in organising such training and any recommendations for potential providers or where to source such training? What problems have been found and lessons learned?

Spencer I used the Keil Centre a couple of years ago for Incident investigation. They were some of the most professional trainers that I have met http://www.keilcentre.co.uk/Default.aspxt Steve
