Hello all

Can I please ask for some advice?

I'm trying to source a database for recording of all accidents / incidents / hazard reporting, which will give me appropriate stats and management reports. This needs to be able to record both primary and underlying causes, and also investigation results.

I already have a robust reporting and investigation system (including forms) in place, thanks.

Our business includes clinical procedures (eg prescriptions, urine sampling and testing, needle exchange) so the system needs to be flexible to incorporate many topics.

I've had no luck so far Googling or on the major sites, so will likely have to purchase a system. Do any of you have any recommendations for systems - both free and at a cost?
